When you are regularly treated by different healthcare professionals, it can be challenging to keep track of all your health information.
The Australian Government’s personally controlled electronic health (eHealth) record system will help you take control of all your health information and help your doctors to provide you with the care you need, this includes the creation of a shared health summary that can be written by a healthcare professional such as your GP. A shared health summary will be an overview of your health, such as any allergies you have, details of medications you are taking and immunisations you have had – key information that doctors and nurses need to know about when treating you and making decisions about your healthcare.
Register now for an eHealth record you can do it in one of four ways :
ONLINE – visit www.ehealth.gov.au
OVER THE PHONE – call 1800 723 471 and select option 1.
IN PERSON – visit a department of Human Services centre offering Medicare Services.
IN WRITING – Complete a registration application form, available from Medicare office or www.ehealth.gov.au or ask at our Reception for this application form and post it to Personally Controlled eHealth Record Program GPO Box 9942 HOBART TASMANIA 7000
